CASE MANAGEMENT FOR PEOPLE WITH DISABILITIES
A case manager helps people with developmental disabilities gain a high quality of life by assisting them in getting the support they need.
If you, or someone you care about, has a developmental disability, there are programs to help you find work, a place to live, medical care, training, financial assistance and many other things.
Your case manager meets with you and your family to help you decide which programs you want. They will explain how each program works and refer you to the ones you are interested in.
Sometimes you can get the support you need without a case manager. However, if you want to be in a program that is paid for by the Developmental Disabilities Program (DDP), then you will need to have a case manager.
What can a Case Manager do for me?
- Help you solve problems.
- Help you understand how the many different government agencies operate.
- Make sure a program meets your needs.
- Make sure you have the medical care you need.
- Help you and your family plan for life changes (like when you leave school or retire).
- Refer you to other resources.
Am I eligible for programs?
The State of Montana’s definition for developmental disabilities is not the same as other states. To get a case manager or to enter an Adult Developmental Disabilities Program, you must:
- Be at least 16 years old;
- Have a psychologist provide formal tests to verify that you have a developmental disability that is due to “mental retardation, cerebral palsy, epilepsy, autism, or other
similar conditions; and
- This disability must have started before the age of 18 and is expected to continue indefinitely.
A case manager will help you collect the information you need to submit to the Developmental Disabilities Program for their review. They will determine your eligibility.
How much does it cost?
You do not have to pay for the case manager.
Are there waiting lists?
MANY PROGRAMS HAVE LONG WAITING LISTS. It is best to contact us early!
Moving from High School to Adult Services:
When a student turns 16 years of age, a case manager can be contacted. If you qualify, the case manager will join with you, your family, your school, and other programs to plan for a smooth transition from school to an adult life.
